JOB TITLE: RURAL COMMUNITY HEALTH WORKER

HOURS: 1 x Full time position (**Apply on the website** hours)

LOCATION: Duhallow area of North Cork , IRD Duhallow, James O?Keeffe
Institute, Newmarket

ROLES AND RESPONSIBILITIES OF THE EMPLOYEE

* To enhance existing services and support the development and
delivery of a Community Health Programme locally which is modelled on
a Community Health worker role and that of a Social Prescribing
Service;
* Under the guidance of the Steering Group (which will comprise of
local representation/stakeholders with reps from CKHC HSE) ensure that
project specific outputs, outcomes and the agreed budget are adhered
to in line with the contractual agreement;
* To focus on improving the health and lifestyle of a community
through a series of health focused initiatives that build on existing
services and facilities;
* To support areas of suicide prevention, intervention and
postvention in the rural community;
* To explore ways of mitigating the risk of suicide in the community
i.e. by empowering people to be aware of and encouraged to engage in
activities that contribute to their overall mental, physical health
and wellbeing;
* To seek to inform members of the community with up to date and
relevant information in how to access support services for themselves
or others who are emotionally vulnerable and maybe having thoughts of
suicide;
* To infom the community of the range of mental
health/wellbeing/suicide prevention training modules that are
available both face to face and online;
* To offer people who are bereaved or affected by suicide
information on supports available i.e. via the Suicide Bereavement
Liason Service, Samaritans etc.
* To identify and then address gaps in service provision locally in
a collaborative manner;
* To develop and deliver Community Health Initiatives specific to
the needs of the local population and area.
* To use the approach of a referral based service mainly through the
local GP practices, primary care network staff and the community and
voluntary services linking individuals to specific local activities to
suit their needs;
* To wok from a hybrid model of general community health and that of
the more targeted role of social prescription which will encompass a
level of referral and non-clinical one to one support to individuals
of a non-medical and more holistic nature;
* To link persons into local activities within their communities to
improve their health and well being;
* To undertake holistic needs assessments and work in partnership
with the individuals and groups in the community to increase overall
health and wellbeing.
* To develop of a series of health focused initiatives in
partnership with local community, organisations, steering group etc as
per the needs of the local community and population groups
* To liaise with other Community Health Workers and projects in
terms of shared learning and development of best practice models
* To establish and maintain effective liaison with stakeholders
including health services (e.g Health & Social care professionals, GPs
, PHN, Community Mental Health Teams etc ), voluntary, social and
education resources, attending relevant meetings as necessary and
engaging with Community Health Network Structures as appropriate.
* To asset map the resources and activities available, and to
identify gaps through working in partnership with all voluntary and
community organisations to build a comprehensive database of local
resources to support the service delivery.
* To ensure information on sources of voluntary and community
support is up to date at all times to enable effective and accurate
signposting and linking of individuals with services.
* To work with local services/stakeholders to bring awareness of the
service and its focus on promotion of health and wellbeing in the
local area.
* To Work closely with the service steering group which will include
representation from all stakeholder groups to support the on-going
development, monitoring and evaluation of the programme.
* To provide bi-monthly comprehensive outcome focused reports
detailing the progress of the service;
* To keep records of work and adhere to confidentiality, information
sharing protocols and provide monitoring information as required.
* To work flexibly as required by the service and to take part in
relevant meetings and events to promote, support and celebrate the
work of the service.
* To provide a personalised, responsive and flexible service to all
persons accessing the service.
* To disseminate a range of information on local support services
and and pathways to care
* To introduce and work with local key community partners on the
establishment of the postvention network - being guided and informed
by the following HSE supported resources - Suicide Prevention in the
Community: ?A Practical Guide to community based organisations?
? You are not alone ?Support for people who have been bereaved by
suicide? ? Lighting the Way Cork North ?An information resource
to support people who are bereaved through suicide?
* Carry out risk assessments and manage risks ? in conjunction
with Team Leader ? as required;
* Review with Team Leader progress against outcomes against
performance targets and reflect on practice;
* Undertake staff development activities that shall enhance
individual and organisational performance;
* Maintain and develop relevant skills and work to the highest
professional standards;
* Any other reasonable duties as assigned by the organisation;

PROFESSIONAL QUALIFICATIONS, EXPERIENCE

* A relevant qualification in Community Work / Community Development
/ Community Education / Community Mental Health e.g. a certificate,
diploma or degree in Youth and Community Work / Community Education.
OR
* At least two years? experience relevant to the post.
