* Healthy Communities SOCIAL PRESCRIBING SERVICE

As part of the Sl?intecare Healthy Communities Programme, Gabrielle
O?Keeffe, Head of Service, Health and Wellbeing and Strategy, Cork &
Kerry is inviting Expressions of Interest (EOI) from organisations
with a track record of social inclusion approaches to addressing
health inequalities, to improve physical and emotional health and
wellbeing; to co-ordinate and deliver the following community-based
programmes:

* Community-led Stop Smoking Support Programme - WE CAN QUIT
* Community Cooking Programme ? HEALTHY FOOD MADE EASY
* Healthy Communities SOCIAL PRESCRIBING PROGRAMME in line with HSE
Social Prescribing Framework

Successful organisations will be contracted by HSE via Section **Apply
on the website** Grant Agreement (Health Act **Apply on the website**)
to deliver a specified level of service as part of the Sl?intecare
Healthy Communities Programme.

Previous experience in delivering one or other of the programmes will
be an advantage but is not essential. The geographical area of the
service is ? Cork city north and will include the FOLLOWING
ELECTORAL DISTRICTS AND HEALTH ACTION ZONES:

Knocknaheeny, Fair Hill A, Fair Hill B, Fair Hill C, Churchfield,
Gurranebraher A, Gurranebraher B, Gurranebraher C, Gurranebraher D,
Gurranebraher E, Farranferris B, Farranferris C, The Glen A Mayfield,
Blackpool A

Organisations can apply to deliver an individual programme or more
than one programme. An application form must be completed for each
programme.
