JOB TITLE: OUTREACH WORKER

REPORTS TO: PROJECT MANAGER OF CLONDALKIN TUS NUA

SALARY RANGE: HSE PROJECT WORKER SCALE

PURPOSE OF JOB

The Outreach worker will lead our harm reduction and low threshold
service. This will include drop-in supports, NSP services, community
street-based harm reduction/outreach service to hard to reach,
homeless and active drug users in Clondalkin and manage a case load of
clients. Promote harm reduction and good health/safe sex practice.
Build non-judgmental, supportive relationships with active drug users.

The service provides a range of programmes and interventions across
the continuum of care ranging from low threshold services to active
drug users to a rehabilitation day Community Employment programme.

It is expected in the normal course of events that the role will
evolve as professional and service demands change.

PRINCIPAL DUTIES AND RESPONSIBILITIES

* Facilitate and oversee a daily NSP service.
* Facilitate and oversee our low threshold drop-in supports
* Provide a community street-based harm reduction/outreach service
targeting hard to reach, homeless and active drug users
* Promote harm reduction and good health/safe sex practice.
* Manage a case load of clients providing short to medium term
interventions and supporting referral pathways
* Facilitate individual key working sessions
* Develop care plans for such individuals to ensure a holistic needs
approach along the continuum of care.
* Conduct screenings and initial assessments, comprehensive
assessments to identify needs of service users.
* To keep and maintain effective record systems, regular updating of
eCASS.
* To work as a member of a team in order to fulfil the policies,
aims and objectives of Clondalkin Tus Nua
* To work in a professional and objective manner with individuals
accessing the service.
* To liaise and network with other agencies ? i.e. statutory,
voluntary and community.
* To attend supervision as required
* Attend case management meetings and act on behalf of the service
user as case manager where required.

PERSON SPECIFICATION

_ESSENTIAL CRITERIA_

* Minimum of a level 7 qualification in Community Drugs Work,
Addiction Studies.
* Minimum of 2 years paid experience?working in a community drug
service
* Minimum of 2 years experience of providing onsite and street based
NSP
* Experience working in a low threshold service
* Experience working with hard to reach homeless/drug users
* Experience of outreach/street based Harm reduction service
* Experience in key working, care planning and case management
* Have a thorough knowledge of drug related issues and their effect
and influence on individuals, families and communities.
* Have experience of high quality inter-agency work, involving
community, voluntary and statutory agencies.
* Effective written, oral and interpersonal communication skills,
including IT skills.

TENURE

This appointment is a full time permanent position (subject to
funding).

6-month probationary period will apply.
