PHYSICAL ACTIVITY FOR PEOPLE WITH CHRONIC CONDITIONS (PACC MODEL)

INTRODUCTION

Carlow Sports Partnership, Waterford Sports Partnership and Westmeath
Sports Partnership are jointly leading a project to bring together and
collaborate with relevant stakeholders in order to improve access,
quality and frequency of physical activity opportunities for people
with chronic conditions. This project is funded under the Dormant
Accounts Innovation Fund via Sport Ireland.

We wish to appoint a consultant to facilitate a process of problem
identification, understanding and solving that draws together diverse
perspectives, expertise and knowledge from core stakeholders, subject
matter experts and end users with a view to increasing appropriate
opportunities for people with chronic conditions to be physically
active. The chronic conditions should be shortlisted by the
stakeholders at an early stage of the process to a select few for the
purposes of this project. The consultant will also document the
challenges and learnings from this collaborative process and model of
working. This is phase one of a two phase process. The second phase
will be the piloting of a number of projects based on the phase one
findings.

CONTEXT

There is a lack of a pathway of appropriate opportunities for people
with a chronic condition to be physically active. Physical activity is
an important part of treatment in many chronic conditions as well as
helping to prevent the development of second/third chronic conditions.

There is also a lack of appropriately skilled and qualified staff in
the community however people with one or more chronic conditions often
present on mainstream community exercise programmes. There may also be
specific needs with regards to facilities and equipment for this
cohort.

People from low socio-economic backgrounds and residing in
disadvantaged areas are less likely to be active. These people are
also more likely to have a chronic condition. Older people and people
with a disability are more likely to be less active and may have a
higher risk of having a chronic condition. **Apply on the website**%

The collaborative process and information gathering will enable the

understand the challenges, barriers and system gaps that currently
affect the participation of people with chronic conditions in physical
activity. Several interventions will be piloted based on the project
consultation/research findings.
