living with cognitive impairment, mental ill health, drug or alcohol

RELIEF ASSISTANT SUPPORT WORKER ? (HOURS AS REQUIRED TO COVER ANNUAL
LEAVE/SICKNESS)

expected to work in sympathy with the spiritual principles of The
Salvation Army.

_ABOUT THE RELIEF ASSISTANT SUPPORT WORKER ROLE:_

_Working in a professional role and environment, core responsibilities
include:_

* Assisting the Support Workers with paperwork such as support plans,
risk assessments & the PASS System
* Coordinate the activity monthly calendar and collect all activity
reports
* Organise external activities for the service users
* Attend appointments with service users to the GP, DCC etc when
required
* Conduct daily room checks & responsible for the store room
* Ensuring Housing Needs Assessment (HNA) is completed and processed
by the relevant local authority as appropriate
* Workshop facilitation
* Ensuring effective handovers and information transmission

_ABOUT YOU:_

* A recognised third level qualification to degree level in one of
the following areas: Social Studies, Psychology, Addiction, Youth
Work, Social Care
* Ability to assist in assessments, care planning and client reviews
* Must be available to do night shifts
* Knowledge in care planning/case management
* Commitment to Health & Safety
* Capable of effectively managing challenging behaviour
* Facilitating physical and mental well being
* Diligent and conscientious
* Commitment to effective professional practice
