1. ROLE

This role offers a unique opportunity to work across multiple agencies
within the sector: Depaul, Ana Liffey Drug Project and Coolmine
Therapeutic Community. This post will provide in reach health and
nursing support to service users in homeless and shielding
accommodation and will advise staff and service users on general
health related issues and Covid-**Apply on the website** guidance.

2. KEY AREAS OF RESPONSIBILITY

* As part of the Inreach Health Support Team, the post-holder will
maintain and deliver a high standard of nursing care providing skilled
nursing assessment and intervention for residents across various
services.
* To practice and undertake all necessary nursing assessment,
intervention and documentation in accordance with NMBI Scope of
Nursing and Midwifery Practice Framework and Standards of practice.
* To provide safe, compressive nursing care to a caseload of
individuals across multiple settings including emergency homeless
services, shielding units and other residential settings
* Develop clinical expertise through nursing practice around issues
related to the health and wellbeing of service users living in
homeless services and shielding units. This role includes assessment,
monitoring, intervention and referral to appropriate pathways for the
management of chronic and acute illnesses for people with complex
physical health, mental health and addiction needs.
* To maintain a high standard of professional and ethical
responsibility while working within the scope of nursing practice as
specified by NMBI, organisational clinical governance, job description
and policies and procedures specific to the role and needs of the
setting.
* To advise staff on clinical issues relating to residents and
support the implementation of symptom monitoring, testing and
isolation pathways to reduce transmission of Covid **Apply on the

* To support service users and accommodation providers to implement
and maintain Covid-**Apply on the website** guidance within their
services.
* To establish care priorities in line with appropriate nursing and
medical standards/guidelines.
* Maintain a high standard of care and continuity with individuals
in relation to their health and wellbeing and refer to appropriate
services as necessary.
* To advocate, coordinate and liaise with health and social care
providers e.g. Community GPs, Primary care teams, Homeless Health
services (GMQ, Depaul Health Initiative, ALDP, Safetynet), mental
health services, secondary/tertiary health care facilities, addiction
services, harm reduction services, peer advocacy services (this list
is not exhaustive)
* To establish and maintain effective working networks with other
partner agencies working within the health, homelessness and addiction
sector.
* To actively participate development and implementation of best
practice in line with Clinical Governance with regards to homeless
health service delivery.
* To collaborate, develop and document appropriate care in
conjunction with Health Support Team members and partner agencies. To
support the ongoing care and case management provided both partner
agencies and accommodation providers.
* Assisting and supporting service users to attend medical
appointments and liaise with other medical services
* To maintain open communication with staff using localized
communications protocols (Handovers, clinical documentation and IT
systems)
* To observe Health & Safety Procedures in particular regular review
of activity risk assessments and maintain a safe working environment
for staff and service users.
* To observe clear professional boundaries in relations with the
service users we work with.
* To undertake and maintain all core training as provided by the
organisation commensurate with the role.
* To support Medication Management procedures within non-clinical
settings where necessary and provide clinical support and oversight to
service managers for the implementation of these policies and
procedures.
* To support case management and resettlement plans for individuals
within PEA and shielding units.
* To respect service users right to confidentiality in our
relationship with them as per the Organizational Confidentiality
Policy and Code of Practice.
* To undertake your role in a professional manner maintaining a high
quality standard of work, and work in accordance with the aims,
values, and ethos of the organization.
* To undertake any additional duties that may be required which are
commensurate of the role in consultation with your line manager.

_ADMINISTRATIVE RESPONSIBILITIES_

* To keep and maintain accurate and up to date medical records in
relation to nursing care, care planning and other interventions
* To attend and participate in weekly team meetings, training,
webinars/conference calls, supervision and other activities as
directed by the organization or HSE Homeless Covid Lead.
* To participate in data collection, reporting, and research
initiatives as directed

_CLINICAL RESPONSIBILITIES_

* Basic Nursing assessment, intervention ad care-planning ie. wound
care, observations, care planning, pharmacy and prescription
management, medication management
* Health Promotion and Screening interventions: seasonal flu
vaccination, Blood borne virus/STI testing, National Screening
services, Naloxone and overdose awareness, Community Hepatitis C
Treatment programmes
* Participation in Covid-**Apply on the website** contact tracing,
testing and sweeping of residential facilitates as directed by public
health and HSE Covid Response team protocols and procedures.
* To support remote symptom monitoring of Covid-**Apply on the
website** as directed by HSE Homeless Covid Lead and other clinical
services.
* To support appropriate assessment, transfer and hospital discharge
info shielding units and implement appropriate care plans to support
resettlement.
* To promote, support and monitor crisis alcohol and benzo detox,
drug stabilisation, and provide appropriate referral for residential
treatment options in conjunction with dual diagnosis worker and GPs.

The above list is not exhaustive; additional areas of responsibility
may be added over time and flexibility to cover for other staff roles
is required from time to time.

3. PERSON REQUIREMENTS

* To establish and maintain effective working networks with other
partner agencies working within the health, homelessness and addiction
sector.
* Have at least 2 years? experience post registration working in
the community, primary care or residential setting desirable.
* Have active registration with NMBI
* Have a good standard of education.
* Have an understanding of harm reduction and low threshold working
and working within these values as part of a multidisciplinary and
multiagency team
* Understand complex health and social factors related homelessness
and addiction supports can be offered in services to service users
with complex needs
* Understanding of risk management principles.
* Capable of responding calmly in a crisis and use your own
initiative to deal with incidents as they arise
* Be able to work as part of a team understanding the importance of
team communication
* Have an understanding of Health and Safety and the health and
safety requirements within a residential setting working with homeless
people.
* Have a knowledge and understanding of the vision, mission and
values of the organization.
