The D**Apply on the website** ACT initiative is a new and exciting
service, bringing together a number of addiction services in Dublin
Northeast. Its purpose being, to initiate a dynamic case management
service to support people with problematic drug and alcohol use and to
target those hardest to reach in Dublin **Apply on the website**.

WE ARE NOW RECRUITING 2 ASSERTIVE CASE MANAGEMENT PRACTITIONERS.

POSITION;

The person(s) will be required to work as part of the D**Apply on the
website** ACT team. This initiative is managed by the Team Leader and
overseen by a steering committee comprising of managers of existing
frontline service providers, the HSE and DNEDATF. The main focus is to
provide a case management service to adults and young people aged

affected/impacted by drug and alcohol use and those hardest to reach.
A key focus of the D**Apply on the website** ACT is to reduce the risk
of harm to the target group and actively promote alternative pathways
through the case management process. This role will complement the
existing community drug and youth services in the area. The team will
work in line with the ethos that underpins these existing services. A
key part of the role will be to capture and collate the information
required for the monitoring and evaluation of the effectiveness of the
initiative to support future funding applications.

THE SUCCESSFUL CANDIDATE WILL HAVE:

* A Third level qualification in the field of addiction,
rehabilitation, social care and/or equivalent.
* Experience of working within a community based
addiction/rehabilitation service.
* The successful candidate must be able to clearly demonstrate
excellent communication and listening skills.
* The candidate must have excellent I.T. skills and ensure that all
records, policies and procedures and its clients are kept in line with
GDPR and good practice guidelines.
* They will have an excellent knowledge of boundaries and
confidentiality.
* Experience in eCASS and HRB would be an advantage.
* Minimum of 3 years? experience working with the target group and
outreach work would be a distinct advantage.

HOURS:?**Apply on the website**.5 hours per week organised between
**Apply on the website**am to 8pm, some Saturday work will be
required.
