MANAGER: HEART SERVICES DEVELOPMENT MANAGER

The Irish Heart Foundation (IHF) is Ireland?s national charity
dedicated to fighting heart disease and stroke. Every hour someone in
Ireland suffers a stroke and every day hundreds of people are
diagnosed with heart conditions. The lives of these people are often
cut tragically short. Many are left with disabilities. Almost

year in Ireland making cardiovascular disease one of the nation?s
biggest killers. We work to turn this around ? support people to
live healthy lives and avoid cardiovascular disease, but also to
support people living with heart conditions and stroke.

THE ROLE:

We need an enthusiastic person with relevant experience to cover a
Maternity Leave contract for The Heart Support Programme Officer role
in The IHF Patient Support Services Team.

KEY RESPONSIBILITIES:

* Responsible for the daily running and overall coordination of
content and monitoring of the IHF heart support Facebook group.
* Ensuring consistent communication and follow up with heart
members, checking in, signposting to all heart supports and
counselling where appropriate.
* Working closely with the patient services management team, help to
design processes and policies that maximise benefit of the page to
heart patients.
* Overseeing the distribution of information and discharge packs to
hospitals around the country.
* Promoting our heart support services to patients and professionals
in conjunction with the Patient Services Development Manager.
* Involvement in the strategic development of services that maximise
benefit to users
* Direct contact for all volunteers involved with heart programmes
and Facebook group e-volunteers. Ensure communication lines are open
and volunteers are met regularly to ensure all is working well for
them.
* Link in with National Volunteer Development Manager on regular
basis for Heart Network volunteer updates or when any problems arise.
* To assist with the creation and dissemination of annual heart
patient surveys.

* Working with The Heart Programme Manager in implementing additional
HSE SLA objectives for HF supports in the following areas:

* Co-ordination of newsletter design, print and distribution.
* Assisting in the organisation of monthly Zoom meetings,
facilitating where needed, liaising with the expert speaker on
practical setting up of session and ensuring links sent to all
consented members.
* Daily coordination of all heart failure referrals from heart
failure clinic nurses, including adding to the database, and reporting
back to nurses where necessary after introductory call.
* Ensuring initial introductory call made to each referral in
accordance with processes created and signposting each client as
needed.

* Working with the Service Development Manager on the development of
other heart support services in areas such as cardiomyopathy, ICD,
Long QT Syndrome, Amyloidosis and SCAD.
* Manage and create all content development for information
leaflets, digital information and online support meetings.
* Identify and communicate gaps of support that are coming up across
the Heart Support Network.
* Create short online polls and evaluate feedback from all groups
including Heart Support Network.
* Maintaining accurate records of invoices, spending and counselling
sessions. Ensuring correct restricted incomes codes applied to each
area of spending and linking in with other teams where budgets are
shared to ensure information is kept up to date.
* Collating data and reporting on numbers of attendees to sessions
and registered members.
* Joint involvement in creation of quarterly reports for HSE.
* Planning and implementing the sign-up journey such as Mailchimp
and website registration form.
* Attending meeting with the Heart Failure Council and patient panel
groups, ensuring meeting minutes taken and sent to all attendees in a
timely manner.
* Involvement in the evaluation of patient support services and
assessment of the impact to patients.
* Data analysis ?use Google Analytics and Facebook insights to
report on and keep up to date reports on the dedicated HF page,
Facebook group and in the future other possible dedicated pages for
other heart conditions.

SKILLS AND EXPERIENCE REQUIRED

* Experience in moderating and content creation on social media
platforms is essential
* Experience in rota management is desirable
* Experience working with volunteers and with people living with
chronic health conditions is desirable but not essential.
* Excellent computer and administration skills and a high level of
proficiency using the Microsoft Office suite, particularly PowerPoint,
Excel and MS Word are essential.
* Previous experience working with a CRM or patient management
database is desirable.
* An ability to work on own initiate as well as part of a team.
* Experience working and networking/signposting to other healthcare
professionals and organizations providing support services for people
living with chronic health conditions is desirable.
* Experience in assessing client?s needs is desirable, but not
essential.
* A reliable and flexible approach.
* A strong client centered approach.
* Excellent organizational skills
* Experience working with budgets
* Confident and comfortable communicating by phone with people
living with chronic health conditions.
* Experience and ability to communicate effectively with a range of
internal and external stakeholders, including Healthcare
Professionals.

EDUCATION

* A 3rd level qualification or equivalent is desirable

This is a full-time permanent role, Monday to Friday. The role is
based in the Irish Heart Foundation?s offices in Rathmines, Dublin.

Currently IHF staff are working to a blended approach, part of the
week is spent working from home and the rest of the week is in the
office. The successful candidate will be supported to do this.
